Forms

This section contains all the forms you are likely to need as a Senior Section member. If you are also an adult member within Girlguiding UK, you can access more forms from the secure members' area.

A review has taken place of all the forms we use in guiding, and new streamlined forms are now available for you to start using.

Residential events:

Senior Section members who hold or are working towards a Permit no longer fill in the A/P form. They now use the same Residential Notification form as adult members. The space on the form that a Commissioner would normally complete can be completed by your Senior Section Leader.

Residential Notification form: To be completed by members planning a residential event.

Information and Consent for Event/Activity form: To be completed by parents of members under the age of 18 attending an event or activity.

Health Information form

International events:

Health Information for International Travel form: To be completed by participants of events involving international travel (in place of the Health Information form).
