Tell us a bit about the session you are recording

This form is to be completed by the lead peer educator for every session. If you have delivered the session by yourself, this is you. If you have delivered this session in a group, only one person should complete the form.

If you are the lead peer educator, you have a responsibility to make sure that the other peer educators in your team have access to the Peer Education Expenses Form so they can claim money for any resources they buy, or travel spend they have for the session. You can download the form once you’ve submitted your answers to the Peer Education Session Log.

What will you do with my data? 
It’s simple. We need the information you share with us to run our exciting activities and to satisfy our legal responsibilities. We’ll keep it safe for as long as  you or your daughter is an active member. 

 We promise we’ll only share your information if: 
– you ask us to 
– the law requires us 
– in order to comply with our policies so you or your daughter can enjoy an activity safely 
– we carry out market research 
– it’s in the public interest 

Don’t worry – we’ll never sell your data or share it for any other reason.
